Understanding the Standard

What Is Lockout/Tagout — and Why Does Your Facility Need It?

Lockout/Tagout (LOTO) is a federally mandated safety practice governed by OSHA standard 29 CFR 1910.147. It requires facilities to establish written, machine-specific procedures for controlling hazardous energy during the servicing and maintenance of equipment. When done correctly, LOTO prevents unexpected energization — protecting workers from serious injury or death.

Machine-Specific Procedures Are Required

OSHA requires a separate written procedure for each qualifying piece of equipment — generic instructions do not meet the standard.

Text-Only Procedures Are Insufficient

Visual, graphical procedures that pinpoint exact energy isolation points are far more effective — and better received by OSHA inspectors.

Annual Audits Are Also Mandated

Beyond initial compliance, OSHA 1910.147(c)(6) requires an annual periodic inspection of each energy control procedure — conducted by an authorized employee.

LOTO Fast Facts

Key figures every safety manager should know

  • LOTO violations are consistently among OSHA's Top 10 most-cited standards every year
  • Failure to develop and use written procedures is specifically cited under 1910.147(c)(4)
  • Facilities with 10+ machines often have significant gaps without a formal audit
  • A single OSHA citation can exceed $15,625 per violation — willful violations up to $156,259
  • Quality Lockout has been writing compliant LOTO procedures since 1994 — over 30 years of expertise

The Risk of Non-Compliance

What Happens When LOTO Procedures Are Wrong — or Missing?

A common error when dealing with Lockout/Tagout is making assumptions about which equipment requires procedures. Not having full knowledge of this OSHA requirement can lead to fines or injuries at your facility.

01

Costly OSHA Citations

Missing or inadequate machine-specific LOTO procedures are among the most frequently cited OSHA violations — resulting in fines that can reach tens of thousands of dollars per incident.

02

Worker Safety Incidents

Outdated, text-heavy, or assumption-based procedures leave employees unable to locate energy-isolating points — dramatically increasing the risk of serious injuries during maintenance.

03

Incomplete Program Coverage

Many facilities assume they have full coverage — only to discover during an OSHA audit that entire categories of equipment were overlooked. Total compliance requires expert identification of every piece of qualifying equipment.

Common Questions

Frequently Asked Questions

Not every machine requires a written procedure — but many more do than most facilities assume. OSHA allows an exception only when the machine has no potential for stored energy and uses a single, easily identifiable energy source. Our experts identify which equipment requires a procedure and which qualifies for the exception.

Our service includes a full facility walk-through and equipment identification, graphical machine-specific LOTO procedures for every qualifying piece of equipment, fabrication and physical installation of custom energy source tags, complete compliance documentation, and a creation template so your team can maintain the program as your facility grows.

The timeline depends on the size of your facility and the number of machines requiring procedures. During your consultation, we'll assess your facility's scope and provide a realistic timeline. Most facilities are brought to full compliance efficiently with minimal disruption to normal operations.

A facility contact should be available to provide access to equipment areas and answer questions about specific machines. Our specialists handle all the technical documentation work — we keep the time demand on your team to a minimum.

No. Quality Lockout does not require any subscription. Our procedure writing service is a one-time engagement. You receive the creation template so your team can add procedures for new equipment internally. When you're ready for an OSHA-required annual audit, we offer that as a separate service.

Yes. Our procedures are written specifically to meet the requirements of OSHA 29 CFR 1910.147. We identify every piece of equipment that requires coverage, document every energy source, and deliver complete compliance records — giving you confidence during any scheduled or surprise OSHA inspection.
